Great source for WDW menus: 

When you make priority seating arrangements at WDW, call 407-WDW-DINE (407-939-3463) up to 180 days in advance for most restaurants.

You should also e-mail about your upcoming trip and reservations.  Let them know the dietary restrictions you or your child have.  There is a form that they can give you to fill out and return to them, which will list all of the reservations you have, foods that need to be avoided and foods that can be eaten.  They will, in turn, forward the info to the chefs at the restaurants and may ask them to call you in advance (or you can also ask that they do so).

(Note:  Rainforest Cafe, T-Rex, and some restaurants at Epcot's Word Showcase are not Disney owned.  We found the chef at Rainforest Cafe in Animal Kingdom to be amazingly helpful and accommodating.  They also only charged us for a child's portion there instead of an adult portion as they do in Disney restaurants.  Also note that Rainforest Cafe and T-Rex are also not part of the Disney Dining plan.)

Kitchen facilities are only available at the Disney resort villas (all of the vacation club locations, which can be rented by non-members), Ft Wilderness cabins, as well as at a surprising location, All Star Resorts, which now has family suites with kitchenettes (sink and microwave, but no stove)!   Those are only in All Star Music right now, but I believe I read they may be added to the other All Star locations in the future.
